Referrals to the Assessement and Referral Team have now closed.
The temporary funding provided by the Commonwealth Government to deliver the Disability Connect and Outreach Program is coming to an end.
The Assessment and Referral Team team will continue to finalise work with all eligible client referrals that were submitted to the team prior to 31 March 2022.

The National Disability Insurance Agency directs all new queries about access to the National Disbaility Insurance Scheme to your local NDIS Partners in the Community who deliver local area coordination supports for eligible Queenslanders with disability.

The Assessment and Referral Team (ART) has been assisting people with disability to apply for the NDIS. The free initiative has been made available through a collaboration with the Department of Seniors, Disability Services and Aboriginal and Torres Strait Islander Partnerships and the Commonwealth Government.
